The Crockett Psychotherapy Website (the "Website) is owned and managed by Stephanie Crockett, PsyD (aka "Dr. Crockett"). Dr. Crockett makes every reasonable effort to keep your personal information protected and private. Information Collected
Dr. Crockett may collect two types of information from users of this Website: 1) Personal Information (your name, email address); and, 2) Non-Personal Information (e.g. cookies or IP address).
Personal Information
“Personal Information” is defined as information that identifies you personally, such as your name, address, phone numbers, or email address. Dr. Crockett only collects and stores the Personal Information that you have provided directly. For example, if you email Dr. Crockett to schedule a consultation meeting, she will collect your contact information and book an appointment through Google Calendar and enter your information into her HIPAA-compliant electronic health record, SimplePractice.
Please note that Personal Information that you are giving to Dr. Crockett is voluntarily, and by you providing this information you are giving consent for Dr. Crockett to use, collect and process this Personal Information.
Retention and Deletion
Dr. Crockett will retain your Personal Information for the minimum amount of time necessary to provide you with the information and/or services that you request. Dr. Crockett may include certain Personal Information for longer periods of time if necessary for legal, contractual and accounting obligations.
Sensitive Information
Dr. Crockett asks that you not send or share any sensitive Personal Information (e.g., Social Security numbers) over email or other non-secure methods. Once psychotherapy services are established, separate privacy and practice policies will be reviewed in the context of a confidential, psychotherapeutic relationship.
Anonymous Data Collection and Use
To maintain the Website’s quality, your IP address may be used to help diagnose problems with the server and to administer the Website by identifying which areas of the Website are most heavily used, and to display content according to your preferences. Dr. Crockett realizes there can be incidents of misuse or unauthorized program incursions, as almost every website, service and user encounters. In those instances, the goals are to move quickly to isolate the problem, ensure or restore proper functionality and minimize any inconvenience to our users. Dr. Crockett may contact you with information that you provide based on these lawful grounds for processing:
Consent. Dr. Crockett may contact you if you give your clear, unambiguous, affirmative consent to contact you.
Contract. Dr. Crockett will contact you under our contractual obligation to deliver services you purchase.
Legitimate Interest. Dr. Crockett may contact you if she feels you have a legitimate interest in hearing from her. For example, if you send a request for a 15-minute consultation, you will receive an email with my availability and ways to schedule that appointment. Personal Information that you provide to Dr. Crockett is stored internally via Google or a HIPAA-compliant electronic medical record. Your Personal Data will only be accessed by those who help to obtain, manage or store that information, or who have a legitimate need to know such Personal Data.

Dr. Crockett ma retain your Personal Data for the minimum amount of time necessary to provide you with the information and / or services that you requested from us. We may include certain Personal Data for longer periods of time if necessary for legal, contractual and accounting obligations.

Dr. Crockett aims to keep the Personal Information that you share confidential.
